Our CCM program is designed to monitor, manage, and relieve chronic pain through a multidisciplinary approach.

Eligibility for Chronic Care
Who Qualifies For The CCM Program?
CCM patients must have at least 2 different chronic conditions listed.
Examples of chronic conditions include, but aren’t limited to:
- Alcohol abuse
- Alzheimer’s disease and related dementia
- Arthritis (osteoarthritis and rheumatoid)
- Asthma
- Atrial fibrillation
- Autism spectrum disorders
- Cancer (breast, colorectal, lung, and prostate)
- Cardiovascular disease
- Chronic kidney disease
- Chronic obstructive pulmonary disease (COPD)
- Depression
- Diabetes
- Heart failure
- Hepatitis (chronic viral B & C)
- HIV and AIDS
- Hyperlipidemia (high cholesterol)
- Hypertension (high blood pressure)
- Ischemic heart disease
- Osteoporosis
- Schizophrenia and other psychotic disorders
- Stroke
- Substance use disorders

- When patients enroll in the program, we focus on coordinating preventative services, assisting with appointment scheduling and authorizations, and even help to manage their medical records.
- Additionally, we provide patients with a dedicated advocate who serves as a direct point of contact for any concerns that need to be addressed by the clinic or provider.

- Actively participate in your care plan.
- Share accurate and complete health information with the program team.
- Follow the agreed-upon care plan and attend scheduled appointments or notify the team in case of unavoidable absences.

- Access to medication management services.
- Health education and lifestyle counseling.
- Remote monitoring of health conditions, if applicable.
- Coordination of care with specialists, if necessary.
